I PUC students from SSLC Board - submit school attested Marks Card for Minority Scholarship under National Scholarship Portal

KINDLY READ THE INSTRUCTIONS BELOW :-

For Registration, it is mandatory to check our college name as Mount Carmel PU College & our college DISE CODE as 29280602440 & then begin the registration process.

(You are held responsible if you register under any other college name / DISE Code and your application will not be verified by the college office for submission to the Minority Department.)

DOCUMENTATON :
It is compulsory to SCAN all the following documents mentioned below. After scanning, 2 sets of all the following documents to be submitted to the college office within 2 days from the date of online registration.

Kindly sign the application form and on all the certificates.

NO CHANGES in information filled by the Student /Parent will be entertained. Hence the students are informed to enter the correct details according to your documents and check repeatedly the entries made in the application form before online submission.

SUBMISSION OF XEROX COPIES OF DOCUMENTS TO COLLEGE OFFICE AFTER ONLINE REGISTRATION :

2 sets of the following XEROX certificates to be stapled separately according to the order mentioned below.

2 Colour Passport size photographs to be pasted on 2 sets of application form.

1. Online application form
2. Institution Verification Form
3. Self Declaration of Family Income
4. Self Declaration of Minority Community by student
5. Copy of previous years marks card (Marks card issued by the respective Board only (ONLINE PRINT OUT OF THE MARKS CARD WILL NOT BE ACCEPTED )
6. College Fee Receipt (2020-2021)
7. Bank Passbook ( front sheet with all details)
8. Aadhar Card
9. 2 Year PUC students should submit marks card copies of X Std & I PUC
10. Declaration by Student
11. Valid Caste & Income Certificate issued by the respective Tahsildar.

(The formats for Institution Verification Form, Self Declaration of Family Income, Self Declaration of Minority Community & Declaration of the Student is attached for your reference).

Submit 2 sets of the application form & Certificates to the College Office for verification & attestation.
First set of attested application form & Certificates will be returned to you to submit to Minority Office , V V Towers, Bangalore.
Second set of attested application form & Certificates will be retained in the college office .

(The application form will be returned and rejected by the college office if there are any errors in the entries made during online registration.
